Responsibility for the Treatment of Personal Data
Centro Médico Excel (doing business as Hospital Excel) with address at Paseo de los Héroes #2507, Zona Urbana Rio Tijuana, 22010 Tijuana B.C., Mexico, is responsible for the management of your personal data in accordance with this Privacy Notice.
Data that is Collected
The personal contact and identification data that Centro Médico Excel will handle include name, age, sex, occupation, address, email, telephone number (mobile and/or landline) and federal taxpayer registry information.
Sensitive Personal Data
Centro Médico Excel will collect and work with sensitive data regarding the health condition, background, and clinical history, as well as information on lifestyle. Sensitive personal data will be kept and treated with strict security and confidentiality for purposes related to the provision of health services.
Purpose of Using Personal Data
Centro Médico Excel uses personal data in order to provide patients with the medical care that is required in accordance with the Hospital Services Provision Contract, as well as the policies, procedures, protocols and other institutional regulations of the Centro Médico Excel. – Data is incorporated into our patient follow-up database. – Clinical records are integrated. – Data is shared with your treating physician and other referred physicians as indicated by your treating physician. – Provided information and documentation regarding medical care is also shared with third-party payers in general, for the payment of the services of the Centro Médico Excel.
Other Purposes:
– Data is also used for statistical purposes, the improvement of care processes, academic research, in which cases care will be taken not to identify the specific patient. – Your personal contact information such as (address, telephone and/or email) may be used to send information about promotions and information about our services. If you do not want to receive any type of advertising information, or or wish that your data not be used for any of the purposes described herein, it is necessary to send an email to info@hospitalexcel.com.
Video Surveillance
Inside Hospital Excel there are video security cameras which may capture images and/or video of the activities carried out in common areas, for your safety and that of the people who visit us and work within the facilities.
Financial Data
To comply with the aforementioned purposes, when necessary, the following financial data will be collected: bank account data and tax data.
Transfers
Centro Médico Excel may transfer, within and outside the country, the personal data in its possession to subcontracted third parties for the purposes indicated in this privacy notice. The data will be transferred to laboratories, clinics, hospitals, insurers, as well as any company within the same corporate group as Centro Médico Excel (Grupo Excel).
ARCO Procedure and Revocation of Consent
For the exercise of your ARCO Rights (Access, Rectification, Cancellation and Opposition) and/or revocation of your consent regarding the management of your personal data by the Centro Médico Excel, please submit a request to the Privacy Committee at the email info@hospitalexcel.com, accompanied by the following information. 1.- Name and contact details such as your full address (street, interior and/or exterior number, neighborhood (if applicable in your country), postal code, city and state). 2.- A copy of your official identification (valid passport, driver’s license, voting card, professional license or immigration document). 3.- If you are not the person for which the request is being submitted, you must submit a document proving the existence of a representation agreement, in this case, a power of attorney signed before two witnesses, identifying the owner (person who is being represented), the representative
and the witnesses. In the case of minors, the parents or guardians must present a birth certificate and a credential with a photograph of the minor, which can be the one issued by the academic institution to which they attend, an IMSS credential (Mexican citizens only), a valid passport or any other with a photograph of the minor. 4.- Present a clear and concise description of the personal data of which one seeks to exercise any of the ARCO Rights, what is the right you wish to exercise and the reasons for which you wish to carry it out..
Consent: In case you require the revocation or deny your consent for your personal data to be used for secondary purposes, we request that you send it through the email info@hospitalexcel.com within a period of five business days.
